Join the 2024 Stargate Mountain Biking Team

Registration Window is April 1st to May 23rd

Instructions

1) Fill out each of the below forms to complete your registration
*Forms #1-5 are mandatory. Only form #6 “Stargate MTB Bike Loaner Agreement” is optional, only needed if you plan to loan a bike from the team

2) For payments on our forms with Venmo Links:
*In the NOTES section of the Venmo payment, you must include the following:
a) the words “STARGATE MTB”
b) the RIDER’S NAME (in all caps)
c) Indicate what the payment is for (i.e. Registration, Sponsorship, Team Bike Security Deposit, etc…)
*This is for Forms 1, 5, 6 if applicable to complete your submission

3) After payment is complete, get the last 4 digits of the Venmo transaction ID and enter these numbers in Form 1 and 5 to complete your submission
